Sanctuary Personnel, an innovative and committed recruitment agency, has a new permanent position available for a Social Worker within the Adult Care Management Team to work full time based in Waltham Forest.
The salary for this permanent Social Worker job is up to £45,815 per annum.
Main duties:
1. Conduct assessments of vulnerable people’s circumstances and issues to determine intervention/referral to the appropriate service.
2. Plan and implement interventions and action for allocated cases. Monitor and review cases. Assess in more complex cases, with guidance.
3. Provide professional advice and guidance to colleagues and partner agencies supporting vulnerable people and carers.
4. Maintain properly documented case files, records, and information. Prepare standard reports as required.
5. Carry out all duties with an awareness and understanding of the safeguarding requirements within the area of responsibility.
6. Provide advice and support to vulnerable people to assist in completing financial assessments.
7. Proactively develop professional knowledge, skills, and behaviours.
8. Act in accordance with all policies and procedures which apply to the job and understand the reasons for this.
9. All opportunities to prevent, reduce and delay access to social care services are taken.
10. Maximise residents' ability to maintain their independence.
11. Be responsible for ensuring the delivery of high quality and legally compliant social care practice to agreed standards for the staff within the remit of the post.
12. Act as care coordinator under the Care Programme Approach (CPA) and care management process of assessment, commissioning, and monitoring.
13. Ensure the implementation of Making Safeguarding Personal.
14. Ensure that the assessment and support planning process is robust and meets agreed guidance and standards.
Requirements of this role:
1. Social work qualification with registration with Social Work England.
2. Experience in managing social work caseloads and supervising others.
3. Experience of community and/or group work.
4. Knowledge of children’s health and social care systems, agencies, and relevant legislation.
