We are looking for committed and enthusiastic nurses to join our Cognitive Impairment and Dementia Specialism (CIDS) which provides community-based treatment and care to all adults developing cognitive deficits and those with dementia. The role of the Community Nurse is to work in partnership with patients and their families/carers in developing their personalised care plans to enable them to lead fulfilling lives. To provide specialist assessments, reviews and nursing interventions to patients accessing CIDS.

Main duties of the job
The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users by undertaking the following duties:
1. To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higher levels of risk.
2. To work alongside the service user to develop their care plan along the recovery principles.
3. To offer specialist nursing assessment and advice where required.
4. To assess and manage ongoing risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers. To regularly review risk factors and make changes to the management of them as necessary.
5. To contribute to the maintenance and development of the Trust nursing strategy.
6. To ensure that clinical practice is evidence-based and consistent with relevant NICE guidelines and Trust policies and procedures.
7. To provide supervision of junior staff and trainees where appropriate (mentorship and preceptorship qualifications).
8. To act within the NMC Code of Conduct at all times.

* Professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing.
* Evidence of continuous professional development.

* Experience of supervising staff.
* Experience of managing clinical caseload including carrying out initial assessments, treatment efficacy reviews and risk mitigation assessments leading to crisis prevention.
