To do this, we are looking for an enthusiastic individual to join our team with a focus on assessing and supporting older people to maintain their independence in the community. Within this role your main responsibilities would include: Co-ordinating and managing cases, effectively using case recording and file management systems. Undertaking assessment of needs and risks as directed. Contributing to complex assessments of need and risk. Reducing levels of risk to service users and others. The pattern of working hours will be determined for individual remits and on occasions may include evenings. Knowledge and understanding of the older person client group is desirable, together with experience of joint working to achieve common goals. Before confirming your appointment, you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland and become a member of the relevant PVG scheme.
