We are Transforming Adult Community Services in Croydon. Do you have the skills to be part of this exciting transformation?
In Croydon, Health and Social care providers and commissioners have formed the Croydon Alliance Board with the aim of providing a whole system transformation that will deliver the outcomes our over 65's have specified that they want.
We require a Healthcare Assistant to work within the Community Nursing Service to provide efficient and effective nursing care to patients in their own homes. The ideal candidate should have experience working within a Health Care setting, be compassionate and able to communicate effectively with patients.
Main Duties And Responsibilities
* To visit patients in their own home to provide care and interventions as delegated by a registered nurse in accordance with an individualised care plan.
* To provide feedback to the registered nurse, colleagues and other professionals regarding the patient's health, wellbeing and changing needs.
* To manage and prioritise delegated patients visits.
* To be an effective member of the nursing team and work as part of the wider Primary Health Care Team.
* To actively participate in Service Development.

Person Specification
Qualifications
* oNVQ III or equivalent in relevant area of practice or willingness to undertake.
Knowledge And Understanding
* oAwareness of own level of knowledge and competency and of when to seek guidance from senior colleagues.
Desirable
* oUnderstanding the HCA role.
Experience
* oExperience of working as part of a team
* oKnowledge of the provision of care for people with long term conditions and/or end of life care.
Skills/Abilities
* oMust be able to demonstrate clear verbal communication skills with patients, carers and staff at all levels
* oAbility to work without direct supervision, e.g. when in patient's home.
Other Factors
* oCar driver with a current UK licence and access to a car for work purposes.
