A dynamic dedicated nurse is required to complement our current team of staff within our intermediate care bed base. Providing rehabilitation for both step down and step up patients, you need to have a good all-round skill set to support the needs of these patients.
Working as part of a dedicated integrated health and social care team, you will develop a variety of skills. A foundation of basic nursing skills is required.
Main duties of the job
1. Ensuring a high standard of care within the intermediate care bed-based facility.
2. Liaising with the Intermediate Care Sister / Charge Nurse to ensure information regarding patient care and changes to care plans, ensuring continuity of care.
3. Providing mentoring, teaching, and assessment to support staff.
4. Liaising with all health care professionals and other agencies to ensure effective service delivery in the intermediate care facility.
5. Performing comprehensive assessments of patient nursing needs.
6. Assessing, planning, implementing, and evaluating programmes of care according to changing health needs under the direction of the Intermediate Care Sister / Charge Nurse.
7. Maintaining accurate records in accordance with NMC.
8. Attending multi-disciplinary case conferences as the named nurse if appropriate.
9. Communicating and collaborating with other members of the Integrated Intermediate Care Team, PHCT, voluntary and statutory organisations to support individual patient care.
10. Establishing and maintaining effective communication with patients, relatives, and carers in complex and potentially challenging situations.
11. Competently performing clinical skills related to the post.
12. Being professionally responsible for delegated visits under the supervision of the Intermediate Care Sister / Charge Nurse.
13. Working without direct supervision.

Job Purpose:

1. Responsible for work allocations of support staff.
2. Completing monthly supervisions for support staff.
3. Organising and prioritising own caseload.
Leadership
1. Acting as a mentor for pre-registration nursing students.
2. Supporting the development of the nursing team by teaching and assessing members of the team including nursing assistants.
3. Undertaking mandatory in-house training and other training that supports the role.
Clinical
1. Performing comprehensive assessments of patient nursing needs.
2. Assessing, planning, implementing, and evaluating programmes of care according to changing health needs.
3. Maintaining accurate records in accordance with NMC.
4. Attending multi-disciplinary case conferences as the named nurse if appropriate.
5. Communicating and collaborating with other members of the Integrated Intermediate Care Team.
Professional Responsibilities
1. Current NMC registration.

Experience
* Newly qualified nurse with Intermediate Care Interest.
* Experience in Intermediate Care.
Qualifications
* Current licence to practice (or willingness to undertake return to practice).
* Diploma or equivalent.
Job Specific Requirements
* The ability to work flexibly in accordance with service needs.
* The ability to travel independently across the Trust.
