Job Description:
The Erewash South Social Work Team is seeking an experienced agency Social Worker to join our supportive, community-based team of Social Care Practitioners and Social Workers. The role will involve working in the Erewash South location, with the office based in Ilkeston, Erewash.
Key Responsibilities:
Conduct Care Act (2014) telephone and face-to-face assessments and reviews, utilising link worker and champion roles.
Demonstrate high levels of autonomy and accountability with timely recording of interventions to progress overall case management.
Build positive, professional links with partner agencies, including Health, Police, Housing, and PVi sectors.
Identify signposting opportunities to support carers, maximise independence, and promote individual wellbeing with a focus on person-centred, strength-based outcomes.
Provide Duty cover to respond to changing situations, effectively triage and assess incoming work, and support Adults and their carers requiring urgent responses.
Manage a complex caseload while meeting statutory duties in a safe, accountable, and cost-effective manner, addressing Care Act (2014) eligible needs.
Desired Experience and Skills:
Experience in taking the lead on Section 42 Safeguarding and chairing multi-agency meetings (e.g., Multi-Agency Adult Risk Management (MARM)).
Familiarity with conducting Mental Capacity Act (2005) assessments, including Best Interest meetings, COP-DOLs, and court work.
Capability to evidence experience in managing cases that meet local authority statutory duties of care.
Note: Support, training, and experience will be provided for candidates wishing to develop their skills in these areas.

The role requires adaptability, excellent communication, and the ability to respond effectively to the needs of Adults and their carers. Experience in promoting strength-based, person-centred outcomes is essential.
