Main duties of the job
An exciting opportunity has arisen for a substantive Band 7 Senior Heart Failure Nurse Specialist at UHS.
We are looking to recruit a motivated, innovative and experienced Heart Failure Nurse Specialist for the Heart Failure Team, focusing on developing the digital provision of care.
You'll be working collaboratively with multidisciplinary team colleagues across both the hospital and community settings, covering Southampton and South West Hampshire. Responsibilities will include both clinical nursing management as well as leadership in conjunction with the heart failure management team.
You will be providing evidence based, expert clinical care and support for patients with heart failure in the most appropriate setting, this includes both face to face and remote management. You will be integrating and developing patient held digital services (MyMR) to achieve effective and personalised management, education and communication to heart failure patients about their condition.

Job responsibilities
Candidates should have advanced clinical assessment skills having completed a post qualification course in history taking and physical examination and experience of managing the signs and symptoms of heart failure.
They should have an NMP qualification and an understanding of the heart failure medications they prescribe, balanced with knowledge of the often complex symptom burden of heart failure patients.
They will be an effective and caring communicator, ideally with an advanced communication qualification, knowing that they will often need to engage in difficult conversations with patients and their family and carers.
A driving license and ability to travel across sites is necessary.
An interest in developing and understanding digital technology, its application and integration into the heart failure service is required as this will be part of the role.
Skill and enjoyment of both autonomous and team working is essential. Candidates will be role models to team members. They will share skills and develop, manage and teach colleagues.

* Registered nurse on relevant part of the NMC register
* Level 7 Post Grad Cert (Min 60 Credits), Master's Degree within relevant speciality or equivalent knowledge to be assessed through evidence of relevant speciality training, experience or portfolio
* Specialist training / Qualification in area of practice

* Evidence of experience and completion of all band 6 competencies within relevant speciality
