You will: Work alongside the Release & Transfer Team and Reconnect Service to ensure that patients are fully supported and needs are met ready for their release, or transfer to another establishment Work closely with patients onsite and other agencies to support the safe transition of care from custody to community Manage the assessment of health needs, planning, delivery and evaluation of care, including clinical risk assessment Manage key risks associated with resettlement in the prison population, including mental illness, anxiety, challenging behaviours and newly sentenced patients Provide support for complex releases - support in the community, GP / Hospital registrations and appointments etc Attend complex case release meetings as required If you are interested in career development, we have a wide range of opportunities for you to develop your skills and experience, which include both internal and external options for development and learning.
