Working autonomously, the post holder will have continuing responsibility for proactively monitoring and managing individuals on the caseload of the team, and will use clinical reasoning and critical thinking to effectively meet the needs of CHC funded individuals. The post holder will develop knowledge in relation to continuing healthcare, underpinned by theory and clinical expertise, and will work in partnership with other disciplines and agencies, including GPs, social services, community nursing and therapists, care agencies, care homes and mental health services in order to manage the care of people with highly complex needs, with a particular focus on Learning Disabilities and Autism.
