Excellent opportunity for anyone who is interested and committed to rehabilitation in the community. The locality is Redcar Community Therapy Team but you may be asked to support the Middlesbrough Community Therapy team too within the role.
This position is for a permanent Band 6 physiotherapist, 37.5 hours a week, to join the established integrated community Therapy team within South Tees. This post will help support some of the initiatives being implemented in the community such as the Teeswide Falls strategy, integrated single point of access MDT and Primary care networks.
South Tees Hospitals NHS Trust Community Collaborative and Primary Care Network are working together to improve community rehabilitation services ensuring that the local population across Middlesbrough, Redcar and Cleveland receive therapy close to home. As a result, there have been changes to service provision recently meaning that the services can be delivered by the right therapist in the right place at the right time.
Working in the community, you will provide rehabilitation in the patients' own home and group settings, working closely with statutory and voluntary services to promote and maintain independence. You will also work closely with the Urgent Community response team to help step and step down patients to help prevent deconditioning and unnecessary admissions to hospital.
Main duties of the job
The post holder will be expected to have experience in rehabilitation as well as having gained experience across a variety of specialities to help manage patients with various conditions. As services are delivered across Redcar and Cleveland Locality, there is an expectation that you will have the ability to work flexibly and travel independently within the community.
Support and in-service training will be provided to develop your skills, knowledge, and understanding of the role. The post offers development in clinical and leadership skills and the post holder will be supported to attend relevant postgraduate courses.
Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.
