We are looking for a passionate and self-motivated Education, Health and Care Plan (EHCP) Coordinator on a full-time basis (36.5 hours) to work with our students and staff at Hallmoor School. As an EHCP Coordinator, you will be responsible for the coordination of the EHCP review process for our existing learners and those due to join the school at the start of the next academic year. You will also be required to contribute to the co-ordination of exams, data, and assessment related to the young person's short-term and long-term targets.
You will work closely with a range of stakeholders to ensure EHCPs are of good quality and are up to date, completed in a timely manner in line with the SEND Code of Practice. In addition to coordinating EHCPs, the role will take on responsibility for bringing in referrals to the programme. You will confirm all learners accepted onto the programme are suitable, ensuring we support learners to achieve paid work outcomes by the end of the academic year.
