Job responsibilities
* Work as part of a multidisciplinary team providing a high-quality service to children, young people, and young adults with life-shortening and life-threatening conditions with complex health needs, and support their families and carers in the delivery of that care.
* Provide high-quality palliative and end-of-life care.
* Assess, plan, implement, and evaluate individualized holistic care packages without direct supervision over a 24-hour period for the children, young people, and young adults, aged 0 to 25 years, who access the service at St Oswald's.
* Contribute to interagency and inter-professional collaboration and teamwork to ensure that delivery of care is timely, well-coordinated, effective, and appropriate to need.
Person Specification
Qualifications
* Registered Nurse with NMC.
* Evidence of post-registration professional experience and development in caring for children & young adults.
* Child Branch or Learning Disability NMC Registered.
* Teaching qualification.
Experience
* Ability to assess care needs and develop programmes of care for children, young people, and young adults.
* Ability to maintain boundaries.
* Work as a member of a multidisciplinary, multi-agency team.
* Experience of teaching and supporting learners and others.
* Ability to work with others to provide a high level of clinical care over a 24-hour period.
* Experience of working with children with disabilities.
* Experience working with volunteers.
Knowledge
* Current knowledge of working with children, young people, and young adults with life-shortening, life-threatening conditions and complex health care.
* An awareness of palliative care philosophy.
* An understanding of family dynamics.

£33,765 to £36,306 a year base salary plus allowance for unsocial hours.
