Job summary Interviews 27th February or 3rd March This is an exciting opportunity to be part of a constantly developing service delivered by the hospice. We are looking for someone passionate about delivering high quality palliative and end of life care for patients who wish to be cared for in their own homes. The role will be to provide leadership for the team of Health care assistants and coordinators of the personal care team. You will be the first point of contact for all families / carers / professionals phoning and receiving visits from the service. The candidate will need to have excellent communication skills with good telephone and face to face assessment skills. Experience of working with outside agencies and other professionals would be beneficial. Main duties of the job We are looking for nurses with: Excellent communication, team leadership skills and organisational skills. A broad range of clinical skills, and desire to develop their practice and that of others to make a positive impact on those they care for and support. A desire for professional development will be welcomed and there is access to in-house development programmes and external education when identified in our staff development process. Previous experience of working in End of life care, Oncology or life limiting conditions are not essential but will be advantageous. Complete holistic nursing assessments via telephone and when required in the patients home. Identifying potential risks within the home environment to ensure patient and staff safety. Use enhanced communication skills to engage with patients, carers, and relatives, including those with language and cultural differences. Review patients over the phone or in their homes as per service criteria. Triage telephone calls and signpost accordingly. Have a knowledge of common symptoms and anticipate changes at the end of life. Support the timely co-ordination of care calls, to ensure smooth efficient running of the service. Lead the team handovers for HCAs in both personal care and support sit services, ensuring they have all the relevant information required. Provide support and reflection as part of this process. Manage and maintain SystmOne records and ensure all details are correct and up to date. Participate in setting standards and the audit of patient and family care.
