Manage a caseload of young people transitioning from care to independent living. Conduct assessments, developing pathway plans, and working with multi-agency teams to provide comprehensive support. Some responsibilities include:
• Conducting assessments of young people's needs,
• Developing and implementing pathway plans,
• Working in collaboration with multi-agency teams,
• Providing advice and advocacy for young people,
• Regularly reviewing and updating plans in line with statutory requirements and the young person's evolving needs.
A qualified social worker with experience in working with young people, particularly those transitioning from care to independence, is required. The ideal candidate will have strong knowledge of relevant legislation, including the Children Leaving Care Act, and excellent communication, assessment, and report-writing skills.
