18.5 hours per week (Evening and weekend work are part of the requirements of the post)

Your Role
As a Be Well Specialist Wellness Coach, you will work within various settings within a Neighbourhood Team delivering a range of specialist Be Well services. You will be required to support high risk participants with complex needs connecting them with suitable Be Well services and other activities or support in the community. You will engage and develop positive working relationships with partners in Health and Social Care, local community groups and other key agencies to increase participation levels and tackle health inequalities across the borough.
About You
The ideal candidate will be passionate about delivering quality services for individuals with long term conditions and older adults. They will have experience of working with a range of people to deliver high quality activity sessions. They will be able to adapt activity to meet the needs of individuals and have effective communication skills. Experience of working with partners and promoting activity is required. They will be able to motivate themselves and work on their own and as part of a team.
