– Collaborate within a supportive team alongside the Multi-Disciplinary Team (MDT), planning, monitoring, and evaluating work.
– Adhere to Social Work England’s standards, professional values, and code of conduct, prioritizing the autonomy and choice of adults, carers, and families.
– Conduct needs-led assessments, complex care planning, and reviews, and initiate multi-disciplinary assessments as required.
– Provide social work support to individuals and their families, involving them in decision-making, and offering guidance and referrals as necessary to enhance social resilience and prevent hospital admissions.
– Practice according to the Professional Capabilities Framework (PCF) and uphold professional standards, demonstrating a commitment to ongoing development.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
