Job Overview
The post holder will work in the function of Liaison Mental Health Nurse between Birmingham & Solihull Mental Health Foundation Trust and the Acute hospital, as part of the Psychiatric Liaison Team Assessment. Responsibilities include the assessment and coordination of care needs, planning and implementation of evidence-based care to service users and their carers within the acute hospital. The role involves providing professional advice and support to other agencies and members of the multidisciplinary team, delivering a proactive clinical service, improving healthy wellbeing, and reducing the burden on acute hospital facilities through timely discharge and utilizing integrated community pathways to reduce readmission.
Main Duties of the Job
1. Work as a practitioner within the acute hospital whilst liaising with other professionals.
2. Work as a generic Psychiatric Liaison Specialist Practitioner Nurse.
3. Responsible for the assessments, planning, implementation, and evaluation of nursing care within agreed parameters.
4. Maintain effective communication pathways.
5. Promote equality, diversity, and rights in accordance with good practice and legislation.
6. Identify and contribute to the training needs necessary for providing quality evidence-based care.
7. Support senior colleagues in the effective assessment and delivery of the Psychiatric Liaison clinical service.
