Job summary An exciting 12-month opportunity has arisen for a Band 7 Specialist Stroke Physiotherapist to join the Knowsley Community Stroke Rehabilitation service. You will be an integral part of the Knowsley Community CVD Service. Our multidisciplinary team consists of Physiotherapists, Speech and Language Therapists and Occupational Therapists as well as an experienced team of Rehabilitation Assistants. You will be instrumental inplanning, co-ordinating, developing, delivering and evaluating comprehensive rehabilitation programmes to patients registered with Knowsley GP's who have had a CVA. You will provide appropriate support and rehabilitation within the community setting, working across a variety of venues and within patient's homes. The Community Rehabilitation service is in demand and continues giving a high level of service to patients within The Knowsley region. Main duties of the job To provide a service to patients within Knowsley who have had a CVA. Provide appropriate support and rehabilitation within their own residence in the community. To work in conjunction with the team to link in with the local hospital trusts to facilitate the identification and seamless transfer of stroke patients from hospital to community. To be responsible for the case management of patients with neurological dysfunction due to Stroke, ensuring appropriate pathways of care are delivered via the community stroke team and communication via liaison and referral to other agencies as required.
