We are looking for a Band 6 Physiotherapist or Band 5 looking for career progression to join our Community Therapy team. The community is where rehabilitation happens and this opportunity will allow you to apply your skills and really make a difference.
Our team of Physiotherapist's, Occupational Therapist’s, Therapy Assistant Practitioner's and Therapy Assistant's offer assessment and rehabilitation to patient’s predominantly within their own homes. We work with patients to identify rehabilitation goals to improve independence and safety with mobility, function and everyday activities. We have a strong ethos of multi-disciplinary working and work closely with colleagues in the wider team to provide a patient-centred, holistic approach.
You will be welcomed into our friendly and established therapy teams and benefit from access to expert clinicians, receive regular clinical supervision and be able to attend and participate in an in-service training programme with protected CPD time and commitment to career progression and development.
Main duties of the job
* To manage a caseload of Community Therapy patients requiring assessment and rehabilitation.
* To triage and prioritise for your own work area, balancing other patient related and professional demands.
* To provide leadership and training for junior members of staff and Therapy Assistants within the team and, to supervise physiotherapy students on placement.
* To work as an autonomous practitioner, within the multidisciplinary team.
* To refer patients on to health, social care and voluntary agencies, as appropriate following discharge from the team.
