Job overview
We are offering an exciting opportunity for an Advanced Clinical Practitioner to join North Mersey Crisis Resolution and Home Treatment Team.
This role will require expert assessment, planning, implementation and evaluation of care for people requiring urgent support with their mental health.
This post will be based at Broadoak Unit, but practitioners will cover a footprint across Liverpool and Sefton. There may also be an expectation for practitioners to support across areas of Mid Mersey depending on service need.
Main duties of the job
1. To provide strategic clinical leadership in relation to an area of specialism using the most up to date and evidence-based theory.
2. To undertake expert assessment, planning, implementation and evaluation of care for a specific group of service users.
3. To offer expert clinical advice for service users, carers, staff and other professionals.
4. To demonstrate advanced competence in highly complex decision making, assessment and the management of clinical needs within the specialist area of practice.
5. To support and complement the existing medical arrangements.
6. To work in partnership with service users, carers and other agencies to ensure effectiveness of outcomes.
Detailed job description and main responsibilities
CLINICAL/MANAGERIAL
1. To work as advanced clinical lead for an area of specialist practice across a range of services within the Urgent Care.
2. To utilise expert mental and physical health professional skills to provide assessment, diagnosis, intervention and clinical guidance as appropriate.
3. To formulate, implement and evaluate interventions with individuals and their carers where appropriate.
4. To make and receive referrals as an advanced autonomous practitioner.
5. To demonstrate advanced knowledge and skills within a defined area of expertise.
6. To critically appraise, evaluate and apply knowledge of theoretical frameworks and research evidence within a specialist area of practice.
7. To visit service users in the most appropriate setting to meet their needs as required.
8. Refer service users to other members of the multi-disciplinary team or specialisms as their needs require.
9. To use independent non-medical prescribing skills in the initiation and evaluation of treatment plans.
10. To ensure that clinical and non-clinical risk is identified and dealt with appropriately.
