Core Clinical Function: FREED Champion
To manage all referrals accessing the service via the FREED pathway, including initial triage and subsequent allocation.
1. Lead a weekly FREED Huddle within the Community Team meeting bringing together all clinicians involved in the FREED pathway.
2. Assess patients referred to the service and co-produce appropriate treatment plans.
3. Access appropriate evidence-based treatment such as MANTRA, CBT-E, SSCM etc.
4. Continuously review the implementation of the FREED service model and care package, advising the Service Lead on its progress and identifying any areas for improvement or opportunities for further service development.
5. Ensure that required core outcome data are collated and reported back to the FREED network.
6. Proactively engage with the FREED project and join the FREED supervision groups.
7. Hold responsibility for co-ordination and management of the FREED project in providing a safe, effective and therapeutic clinical service, ensuring the delivery of high-quality care packages.
Core Clinical Function: Keyworker
Manage and oversee the care of service users with highly complex needs.
1. Manage a caseload utilising person-centred assessment, planning and implementation of care plans.
2. Evaluate care plans and outcomes including active participation in clinical audits and outcome measures.
3. Work closely with service users and their families/loved ones to ensure their wishes and needs are heard and understood.
4. Work closely with the service users and our support worker to ensure a holistic approach to enhance quality of life and confidence in activities of daily living and integration in the community to prevent isolation.
Core Clinical Function: Eating Disorder Therapist
Assess clients referred to the Community Eating Disorder Service and provide appropriate treatments.
1. Provide evidence-based therapies for people with an eating disorder i.e. CBT, CBT-E, CBT-T, MANTRA, SSCM.
2. Attend the weekly Community Team meetings to discuss cases, offer clinical opinions and support team.
3. Deputise for Service Lead in their absence and represent the team as required.
Core Clinical Functions: All Roles
Produce accurate and timely documentation, conduct and manage the maintenance of accurate and timely documentation/records on all service users in line with Navigo policy.
1. Conduct, supervise and oversee risk assessments of all service users.
2. Provide professional clinical leadership to multi-disciplinary meetings i.e. referral/review/business, professional or safeguarding meetings.
3. Report and document incidents as required and act upon findings as appropriate.
4. Provide practical support, guidance and education for service users, carers and other professionals as required.
5. Be competent in use of relevant screening and assessment tools.
6. Encourage completion of outcome measures to allow for service evaluations.
7. Lead and participate in formulation, delivery and review of individualised evidence-based care/care packages according to NICE guidance including non-medication strategies.
8. Manage and co-ordinate multi-disciplinary meetings i.e. referral/review/business/handover.
9. Provide clinical leadership and professional expertise to the team.
10. Deliver training where appropriate for other professionals.
Management Responsibility
You will be expected to support the work of the Service Lead in managing the performance of the service. This will include resource management, operational planning, clinical governance, and workforce planning.
1. Support and plan new service initiatives taking a lead management role in these initiatives as requested by the Service Lead or operational service manager.
2. Be involved with the review and implementation of any relevant policies.
3. Ensure that the service through the implementation of performance management, all key performance indicators and targets identified by Navigo are met and adhere to local and government initiatives.
