The nurse will assist and work independently within the wound clinic based service, alongside the Tissue Viability Team. The nurse will actively contribute to the management of complex wounds, assessing and treating patients following best-practice guidance. The nurse will deliver training, formally or informally on all aspects of wound care to patients, carers and to all staff groups and multidisciplinary team.

Main duties of the job
* To work within the service with agreed Trust protocols and guidelines.
* To manage own workload.
* To work as a member of the multidisciplinary team, sharing in the responsibility for the provision of 7 day, 24 hour care where this is required.
* To promote and adhere to the principles and practice of clinical governance and audit.
* To demonstrate effective multidisciplinary team working and acknowledge the role of individual team members, ensuring that expertise is seen as a resource within the team and the Trust.
Job responsibilities
* To provide specialist advice of best practice relating to all aspects of wound care in line with both local and national guidance to all staffs, patients and carers.
* To ensure safe practice within the NMC Code of Professional Conduct and adhere to the trusts policies and procedures.
* To actively contribute to the prevention of all pressure ulcers and provide specialist advice to all staffs, patients and carers.
* To establish and maintain effective relationships with patients, carers and the multi-disciplinary team in order to facilitate patient choice and independence.
* Assess, plan, implement and evaluate clinical care holistically.
* Actively support innovations and improvements in practice related to wound care including participation in the assessment and evaluation of wound care products and equipment with the aim of providing cost effective care.
* To ensure personal, professional, and clinical expertise is upheld through regular evaluation and updating of individual practice.

Education/Qualification
* Registered Nurse
* Leg Ulcer management / Tissue Viability or Equivalent
* Teaching and training
* Ability to work with and lead others across profession
* Community nursing qualification experience of working in primary care
Experience
* Experience in Tissue Viability
* Evaluation and audit experience
* Experience in undertaking research application of research to practice
Skills and Knowledge
* Advanced nursing skills in the assessment, planning, implementation and evaluation of patients with Tissue Viability needs
* Experience of working with patients and professionals to prevent and treat pressure ulcers
* Independent decision making with appropriate reference to others
* Some knowledge and understanding of quality improvement, clinical governance principles application in clinical setting
* Evidence of involvement in benchmarking, testing services
