Job overview
We are looking for an enthusiastic and dynamic band 6 Physiotherapist to join our friendly Community Stroke/ Early Supported Discharge Team. If you are a band 5 with relevant experience and are passionate about making a difference with these patients, please consider applying as a band 5 with potential to develop to band 6 in post.
The Community Stroke Rehab Team provides stroke specialist rehabilitation for Enfield residents aged 18+. Care is provided in the patient’s home and local communities dependent on their functional goals. It is a friendly multidisciplinary team with generic rehabilitation assistants to support patients to carry out treatment plans set by the therapists.

The team are based at St Michael's Primary care centre with ease of parking for staff and access to Enfield Borough. It is in close proximity to the M25 and has good transport links to Herts and central London.
Main duties of the job
1. Providing care to a multi-cultural and diverse population, delivering high standards of physiotherapy care to stroke patients and their carers in the community.
2. Performing comprehensive physiotherapy assessments of patients with diverse presentations and complex physical and psychological conditions following a stroke.
3. Using assessments to provide a clinical diagnosis and develop and deliver an individualised treatment programme.
4. Holding responsibility for own caseload and working with minimal supervision alongside other members of the MDT.
5. Undertaking all aspects of clinical duties as an autonomous practitioner.
6. With support and guidance from senior staff, undertaking evidence-based audit and/or research projects to further own and team’s clinical practice.
7. Contributing to health promotion activities within the service and wider Trust initiatives.
8. Providing cover for other physiotherapists in the service as required.

Person specification
Qualifications
* Recognised Diploma/Degree in Physiotherapy
* Evidence of ongoing CPD in portfolio
* CSP Member
* Attendance at relevant Post-Graduate Courses.
Skills
* Ability to prioritise, organise and work in a busy environment
* Able to demonstrate a high level of clinical reasoning.
* Good communication skills, able to present information (written and oral) in a clear and logical manner.
Experience
* 18 months – 2 years post graduate clinical experience.
* Experience of working in stroke/neuro.
* Experience of working in the NHS.
* Experience of working in a community setting.
Knowledge
* Knowledge and understanding of evidence-based stroke care.
* Fluent in written and spoken English.
Other
* Car owner/driver with valid UK driving license.
