Community Practitioner

The Gateshead Learning Disability Team are happy to be offering an exciting opportunity for a Band 6 Community Practitioner.
We are a multidisciplinary community team working across the whole of Gateshead with individuals with complex needs. The successful post holder will be expected to demonstrate a strong commitment to high quality patient care, good organisational skills and be able to demonstrate prioritisation of workload. Key components of the role will be to ensure the service offers a safe, caring, responsive and well-led service.
The role will include liaising with agencies such as social care, service providers, inpatient services, safeguarding, alongside a range of other agencies. Within this team we pride ourselves on having a broad range of knowledge within these selected areas and ability to manage the risks associated with this.
Main duties of the job:
1. To be responsible for the assessment and management of a caseload of patients who have substantial and complex needs.
2. Provide specialist clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public.
3. You will be expected to provide high quality assessment and care, alongside offering the wider team support, advice and supervision.
4. Work collaboratively with patients to develop care plans, recovery, and wellbeing plans. Ensuring patients have access to regular reviews to monitor progress and adapt care plans as necessary.
5. To consider carer needs and ensure carer and family are involved in care planning where appropriate.
6. To deliver evidenced based clinical interventions.
7. The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
8. Coordinate treatment planning, interventions, clinical reviews and discharge planning.
9. Provide clinical supervision to staff as delegated by Team Manager and Clinical Leads.
10. To provide compassionate care that is based on empathy, kindness, respect and dignity.
11. To incorporate a patient centred philosophy into nursing practice and patient centred collaborative care.
Person specification
Knowledge and Experience
* 12 to 24 month band 5 qualified nurse experience dependent upon service need.
* Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
Education and qualification
* Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist.
* Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
* Proven track record of post registration clinical experience relevant to the post.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
* Up-to-date knowledge/experience of relevant legislation.
* Experience of working in a community team.
* Post registration qualifications in clinical interventions.
Skills and Competencies
* Proven ability to carry out and provide comprehensive written assessment of individuals with complex health needs.
* Demonstrate an understanding of effective and appropriate treatment options including a range of experience based clinical approaches.
* Clinical leadership.
