Job summary We have a very exciting opportunity to join our integrated Heart Failure Service at Calderdale and Huddersfield NHS Trust. We are seeking a highly motivated individual with excellent interpersonal and communication skills. The successful candidate will be responsible for the development of a cohesive management programme for patients with heart failure in line with local and national guidelines, working across acute, community and virtual ward settings. The service will operate 7 days a week, with a view to extending hours of 8am to 8pm in the future and requires flexible working within an evolving proactive, dynamic, and committed multidisciplinary Heart Failure Team. Main duties of the job The aim of the role is to work autonomously, within a the team, managing own caseload and to identify patients who can be supported on the Heart Failure Virtual Ward, reducing length of stay, preventing unnecessary admissions to hospital, and improving heart failure management and control with optimisation of evidence-based treatments. Will be responsible for the early identification of patients for the Heart Failure virtual ward from the acute setting and supporting the on-going care for patients whos are actively being managed on the virtual wards.
