Your Role
We are currently recruiting for a mental health hospital discharge social worker to join our dynamic and friendly team. You will be part of a newly created all age adult mental health hospital discharge team. You will work collaboratively with our Pennine Care colleagues and have a crucial part to play in improving mental health services and mental health outcomes for people leaving hospital and their carers. Career progression by completing your practice educator training, AMHP and BIA training will be encouraged.
You will support older and working age adults who also have a functional or organic mental health diagnosis to be discharged from hospital safely and in a timely manner.
This role will include working collaboratively with the person and their carers. Liaising closely with health, advocates and third sector colleagues. You will be co-producing Care Act assessments, support plans and risk assessments. As well as undertaking Mental Capacity Assessments, chairing Best Interests Meetings, writing Social Circumstances Reports and attending Mental Health Tribunals.
About You
We are looking for a social worker who has knowledge of the Mental Health Act and is motivated to get the people of Stockport the right care at the right time whilst adopting a strengths based approach. You will identify community resources and commission appropriate care and support. We are looking for someone who is a qualified and experienced social worker who has a keen interest and passion about mental health social work.
