These include face-to-face consultations, telephone consultations, home visits, triage, checking and signing prescriptions, and dealing with queries in a timely fashion. Perform clinical examinations of patients to assess, diagnose and monitor conditions Diagnose and treat various illnesses/ailments Liaise with other healthcare professionals (e.g., midwives, pharmacists, health visitors and psychiatrists) as part of multidisciplinary teams Contributing to QOF requirements and recording data appropriately. Assisting our Care Coordinators where required. Developing care plans for health-related illnesses. Screening patients for health education.
