Advanced Specialist Pharmacist Pre-Assessment Clinic

An exciting opportunity has arisen to lead, develop and provide the day to day service of the established Pre-Operative Assessment clinic on the Bedford site. We are looking for an experienced clinical pharmacist with excellent personal, communication and leadership skills complemented by a strong background in managing surgical patients.
Day to day activities will include reviewing surgical patients to ensure that their medicines are optimised prior to their elective admission and to ensure that there is a clear medicines management plan in place. The successful candidate will join the established Pre-Assessment team and have the opportunity to optimise the service to best meet the needs of our patients and the Trust. The ability to work in close collaboration with senior medical, nursing and pharmacy colleagues will be essential as will the ability to demonstrate patient focussed and leadership behaviours.
Close working relations with our Luton site and the corresponding Specialist Pre-Assessment Pharmacist will be essential to ensure smooth integration across both sites.
Main duties of the job
To lead, plan and develop advanced specialist clinical pharmacy services for the pre-operative assessment clinic (POAC) and surgical Clinical Service Lines including, where appropriate, non-medical prescribing in line with the agreed scope of practice.
To be a source of expert specialist advice for matters relating to pre-operative assessment and surgery, to other pharmacists and other healthcare professionals.
To support the Clinical Service Line and Pharmacy Department in meeting its risk and clinical governance responsibilities around the use of medicines through effective guideline, policy and SOP development and review.
To undertake clinical supervision of Foundation Pharmacists, Senior Rotational Pharmacists and Trainee Pharmacists to support their development both professionally and clinically.
To act as a professional role model to all registered clinical pharmacy staff, actively demonstrating integrity, ownership and leadership in all aspects of the role.
Job responsibilities
Prescribing Practice
* To provide expert pharmacy advice to POAC patients, including participation in cross speciality MDT discussions.
* Responsible for the appropriate management of patients referred for medicines optimisation / reconciliation input or who otherwise fall within the agreed scope of practice.
* Work with consultants and the pharmacy team to promote best practice in the pharmaceutical care of patients in line with national and local guidance.
Clinical
* Assist the multi-disciplinary team in the pharmaceutical care of each patient by:
* Suggesting alterations to patients complicated medication regimes to simplify and aid therapy compliance.
* Actively participating in clinic related activities and multidisciplinary team meetings providing clinical and financial advice if appropriate.
* Promoting rational, safe and economic use of medicines.
* In collaboration with other Advanced Specialist Pharmacists to jointly provide professional, managerial and clinical leadership to the team of pharmacy staff involved in the delivery of pharmaceutical services.
* Develop, implement and monitor pharmacy services to ensure they are consistent with Trust priorities and requirements.
* To provide the Clinical Service Lines and Clinical Pharmacy Services Manager with regular assurance that the pharmacy service is meeting agreed Key Performance Indicators.
Education and Training
* Participate in the regular clinical training and supervision of Foundation and other grade pharmacists and pharmacy technicians.
* Provide education and training for patients and their carers on the safe and effective use of their medicines.
Other
* To provide information and advice to Trust committees as required.
* To adhere to the policies and procedures of the Bedfordshire Hospitals NHS Foundation Trust as they relate to the delivery of services.
Person Specification
Qualifications
* Bachelor/Master of Pharmacy degree or equivalent.
* Post graduate Diploma in Clinical Pharmacy or equivalent experience.
* Member of the RPS.
* Post graduate Masters in Pharmacy.
* Quality Improvement Training.
* Management courses.
Experience
* Evidence of specialist practitioner practice.
* Experience of influencing members of the multidisciplinary team in delivering patient care.
* Evidence of managing and delivering a project.
* Experience of clinically supervising and training staff.
* Experience of delivering education and training to undergraduates and postgraduates of various healthcare professions.
Knowledge
* Expert knowledge and understanding of relevant standards and guidelines.
* Knowledge of legislation/guidance relating to medicines usage and prescribing.
* Knowledge and awareness of implications of current developments in pharmacy practice and NHS strategies.
