Want to know more? please contact theresa.larcombedorsetgp.nhs.uk Main duties of the job The post holder will work within their clinical competencies to provide pharmaceutical support to all practices across Mid Dorset PCN and work closely with members of wider healthcare teams to improve care and support safe and effective prescribing. The post holder will work as part of a multi-disciplinary team in a patient-facing role. They will take responsibility for areas of medicines optimisation within the PCN and undertake structured medication reviews and support long term condition management to proactively manage patients with complex polypharmacy. The post holder will need to be proactively involved in quality improvement and audit as well as managing some aspects of the Quality and Outcomes Framework. The post holder will be supported to help develop their role and work to improve the care and outcomes for the patients across the PCN. This will include becoming and/or operating as a non-medical prescriber. To play a pivotal role in the delivery of high-quality primary health care within the PCN.
