Braintree Primary Care Network has an exciting opportunity for a hardworking, enthusiastic and innovative co-ordinator to join us on a full or part time basis. The role will be located primarily at the PCN hub in Braintree but the postholder will be required to travel between all of the GP practices within our PCN. The post holder will be employed by Braintree PCN, on behalf of the five member practices, Mount Chambers Surgery, Blandford Medical Centre, Blyths Meadow Surgery, Church Lane Surgery and Silver End Surgery. The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN, though external relations will also be important. The Care Coordinator role will support the PCN management team and member practices in coordinating all key activity including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred. The role will include supporting digital initiatives and includes responsibilities for the co-ordination of the patients journey through primary care. We are seeking a motivated, innovative and experienced care co-ordinator with proven skills to work in primary care to join our professional and friendly multidisciplinary team which includes advanced nurse practitioners, physician associate, clinical pharmacists, paramedics, mental health practitioners, a social prescribing link worker, care co-ordinators, pharmacy technician and PCN management. The care co-ordinator responsibilities include but are not limited to the following: Work with all primary care professionals within the PCN to support patients with appointments, advice and general queries. Work closely and in partnership with the clinical and non clinical teams and a host of other professionals Support the clinical director and member practices in the delivery of the DES specifications, issued by NHS England Help patients, their carers & relatives to manage their needs through answering queries, making, and managing appointments Provide coordination and navigation with the aid of digital tools for people and their carers across health and care services Support the coordination, delivery and administration of MDTs within the PCN Reach out to people who don't find it easy to access care, i.e. to help address health inequalities and to co-ordinate social, debt support and other social determinants of health.
