Job summary This post is being offered as a 6 PA post. An indicative job plan is provided, though the exact job plan will be agreed between the NCA and the post holder on commencement. This post is based out of the Fairfield General hospital, Bury with the post holder will have opportunity to work with a wider MDT and in an Community setting within Bury. The post holder will enjoy a generous 1.0 PA SPA allowance. There are currently no on-call or out-of-hour duties associated with this post, but this may be reviewed in future. The post holder will lead, compliment, and develop Specialist Palliative Care services within the Bury Locality. The post provides an opportunity to enhance Hospital-based services and build on developments such as the Rapid Discharge Pathway, a Palliative Care Link Consultant programme, and Palliative Care input into other MDTs. The post should support the advancement of local Palliative and End of Life strategies with a remit to improve the care of Palliative Care patients across a wide range of settings and work towards the Greater Manchester Palliative and End of Life Care Commitments. Main duties of the job To work in collaboration with the multidisciplinary SPCT to identify appropriate interventions, management plans and opportunities for early advanced care planning for Palliative patients where appropriate to support PPC and PPD. To provide regular Specialist Consultant review of Hospital inpatients, working closely with the Hospital palliative care specialist nursing team and provide additional support to the wider Bury SPCT as necessary. To provide a lead role for the Hospital Specialist Palliative Care Team, BCO Palliative Care Governance & Operational meetings and contribute to Palliative Care MDT. To provide peer support for the existing Hospital Palliative Care Consultants. and other Consultant colleagues in the NCA. To contribute to on-going development of the service model across the Bury locality, in partnership with the Northern Care Alliance, Bury Hospice, Bury Community teams, Primary Care and Nursing homes to realise a fully integrated service and achieve the GM Palliative and End of Life Commitments. To provide training, teaching and support to staff in settings where palliative care is provided to promote an integrated approach.
