Job Overview
This role is focused on supporting people with severe and enduring mental health needs, and their carers (friends/relatives/carers) and supporters in the community. The aim is to enable and assist them in meeting daily health, social care, and wellbeing needs, in line with personal recovery goals, while facilitating engagement with mainstream services.
The post holder will contribute to the ongoing assessment, planning, delivery, and review of activities and interventions against identified health, social care, and wellbeing needs, acting as care/recovery coordinator for an allocated group of service users. The role will also require undertaking and delivering defined activities and interventions for a wider group of identified service users, in accordance with the agreed personal recovery plan.
This may be either on a one-to-one basis or as part of a group activity. Some regular out-of-hours working may be required.
Main Duties of the Job
1. Relevant experience of working in any health, social care, or related voluntary organisation setting.
2. Able to demonstrate some awareness of theories underpinning health and social care.
3. Able to articulate a clear knowledge of policy and legislative frameworks within which mental health services are delivered, including a person's rights under the MHA, CPA, and Mental Capacity Act.
4. Demonstrates a good understanding of recovery principles and the role of secondary mental health services in the delivery of care.
