An exciting opportunity has arisen within the Health Board's Cardiac Rehabilitation Team to deliver a rehabilitation service for acute patients following a cardiac event in the Carmarthenshire area.
Delivering services for patients closer to home in line with the Health Board's strategy 'A Healthier Mid and West Wales: Our Future Generations Living Well' and associated plans, Cardiac Rehabilitation aims on delivering an effective, efficient, and sustainable service by shifting the current focus on hospital-based care and treatment to a focus on services more readily available in local communities.
A key priority of the role is to provide ongoing support and guidance during a patient's recovery following an acute cardiac event. The role will involve contacting patients following discharge, virtually and face to face, and to provide specialist advice, teaching and instructions to relatives, carers, and other professionals, whilst promoting understanding of the aims of cardiac rehabilitation and ensuring a consistent and safe approach to patient care.
The role includes signposting to appropriate services as required. There will then be the opportunity to be involved in delivering education and exercise groups, which will include participation in function capacity testing.
Main duties of the job
1. Clinical responsibility for a designated caseload of patients.
2. To interpret and analyse clinical and non-clinical facts to form an accurate diagnosis and prognosis in a wide range of highly complex cardiac conditions, to recommend the best course of intervention and to develop comprehensive discharge plans.
3. To provide specialist advice, teaching and instructions to relatives, carers and other professionals, to promote understanding of the aims of cardiac rehabilitation and to ensure a consistent and safe approach to patient care.
4. To undertake the comprehensive assessment of patients, including those with a complex presentation, using investigative and analytical skills and to formulate individualized management and treatment plans, using clinical reasoning, and utilizing a wide range of treatment skills and options to formulate a specialised programme of care.
5. To communicate effectively, provide specialist advice to patients and carers to maximize cardiac rehabilitation potential and to ensure understanding of condition.
6. To promote the benefits of an active and healthy lifestyle in primary and secondary prevention of CHD.
7. To promote social integration and reduce social isolation through community-based cardiac rehabilitation.
8. To promote a non-threatening, non-institutional approach to health promotion.
9. To influence behaviour change in those with chronic disease.
