Health Care Assistant Rapid Personalised Care Service
The role of Rapid Personalised Care Service Healthcare Assistant (RPCS) is in place to provide end-of-life care to patients in their own homes in the final weeks of life.
Main Duties of the Job
1. Competently working alone, as required, with patients and families in the home environment with remote support and guidance from the RPCS Management in adherence to lone worker policy and procedure.
2. Working collaboratively with hospice and community partners.
3. Acting as a Rennie Grove Peace Hospice Care ambassador, actively demonstrating our values and positively promoting the specialist care we provide.

Job Responsibilities
1. Demonstrate and maintain consistently high standards of individualised personal care and support under guidance of the RPCS Management.
2. Work alongside the patients' families and/or healthcare professionals, maintaining an approach that incorporates physical, psychological, social, and spiritual aspects of care in accordance with the Rennie Grove Peace philosophy.
3. Observe and report changes in the patients' symptoms, conditions, and circumstances to the RPCS Management and notify other healthcare professionals as necessary.
4. Work in accordance with the patients' agreed care plan.
5. Responsible for identifying and acting on any risk of safeguarding to patients and their families in accordance with the Rennie Grove Peace policies.
6. Keep accurate and legible records in accordance with legislative requirements and Rennie Grove Peace practice and procedure.
7. Assist with the patients' washing, bathing, showering, pressure area care, and oral care.
8. Assist with dressing and undressing the patient as required.
9. Assist with the patients' nutritional requirements including preparation of light meals and drinks according to dietary requirements and choice.
10. Assist with a patient's toileting, which may include the use of bedpans, commode, or lavatory.
11. Prompt medication if dispensed in a pharmacy blister pack.
