His is an exciting time to join RDASH & in particular the Doncaster Care Group, as one of the Consultant Psychiatrists serving Older People's Mental Health services. You will have the opportunity to work flexibly across the community mental health teams, the enhanced care home team, as well as the young onset dementia service. You will work closely with 2 consultant psychiatrists, two nurse consultants, a substantive specialty doctor & multidisciplinary teams.
The older people’s services in Doncaster cover two main localities, each combining two sub-localities, each served by a community mental health team (CMHT). These are North, South, East & Central. The post holder will fulfil a flexible role across all of the teams & offer peripatetic cover for leave for consultant colleagues & when activity demands.
Each of the 2 main localities are served by a Consultant Psychiatrist, a Nurse Consultant & a full multidisciplinary team. A substantive SAS doctor covers two of the sub localities. The older adult CMHTs liaise regularly & closely with the one 20 bedded inpatient unit (Windermere) located on Tickhill Road to ensure safe flow of all patients into & from the unit. The inpatient unit also has a dedicated nurse consultant for all organic inpatients & a full MDT including psychology input. The Consultant Psychiatrists follow their respective patients through from the CMHT to the inpatient unit & upon discharge to ensure flow patient & safe continuity of care.
• • The post-holder will be expected to work in a flexible manner, supporting all multi-professional teams as per determined need & deliver care across the range of older adult community and enhanced care settings served. This is a peripatetic post, where the postholder will offer cover into teams for annual & study leave & other determined circumstances as per clinical need and acuity.
• The postholder will work into the enhanced care home team and support the nurses on diagnosis & management of patients in 24hr care. In addition, the post holder will also work with the young onset dementia service.
• The post holder will have continued responsibility for the care of the patients in his or her care, which include the administrative duties associated with the care of the patient & the running of the clinical team.
• The post holder is expected to work in both organic & functional aspects of mental health services for older adults in the community. The post holder can case manage their own, functional inpatients.
• The postholder will provide flexible medical leadership, consultation, advice & guidance within the community mental health teams, ensuring that care & treatment are safe and effective, efficient and within national guidelines.
• The postholder will provide medical assessment, diagnosis & formulation to appropriate patients. On completion of assessment, advise on diagnosis and treatment within the teams will be delivered and communicated to relevant referrers.
