As part of the phase 3 cardiac rehabilitation team, the post holder will be responsible for providing an exercise programme for patients with a diagnosis of coronary artery disease or myocardial infarction, undergone cardiac intervention or surgery, or those with a diagnosis of heart failure.
The post holder will assist the cardiac rehabilitation nurses and team lead in the delivery of the cardiac rehabilitation service. He or she will promote continuous development in clinical practice and patient care; encourage innovation and appropriate change in service delivery within the cardiac rehabilitation service in response to patient need, clinical audit and evaluation, and national guidance.
Main duties of the job
Working in collaboration with cardiac rehabilitation Nurses to assess activity levels and cardiovascular risk factors and to provide them with exercise and health education advice and an individualised activity and exercise programme.
Record patient data and maintain the patients' clinical record on Health Machine and facilitate input of data on the NACR audit. Assess with the Cardiac Rehabilitation nurse the suitability and medical stability of all patient's exercise tolerance and advise on the rehabilitation process and exercise capacity, and risk stratify individuals accordingly.
Conduct risk stratification for each patient and consider this when designing the patient's programme.
