Job summary This is an exciting opportunity to work in the community within a multi-disciplinary, integrated team in theSouth Kent Coast and Thanet area.As a Senior Community Learning Disability Nurse you will be part of a proactive and supportive LearningDisability Nursing Team enabling and facilitating people with a learning disability to meet their health needs.You will facilitate access to mainstream health services, working collaboratively with primary care, acutehospitals, and other community health providers. You will be actively involved in promoting the individual'sindependence and understanding of their health needs, working to reduce health inequalities andimplementing reasonable adjustments. You will hold a caseload which you will manage under clinicalsupervision. You will have line management responsibilities for band 5 Community LD Nurses/Nursing SupportWorkers and provide supervision and support the development of other team members, including responsibilityfor supervision and assessment of Nursing Students/Apprentices on placement with the team. You will useyour clinical judgement to undertake timely evidence-based assessment, planning, implementation andevaluation of holistic nursing interventions on a wide range of needs including end of life care, dementia,sexual health, behaviour, complex care, transition, desensitisation, and health education and promotionincluding development and delivery of group work.
