Dougie Mac Dementia Services offers a 1:1 Dementia Support service, in which Dementia Support Workers are paired with people with dementia to enable them to participate in social activities suited to their needs. This service aims to engage people with dementia in 1:1 activities to facilitate wellbeing and enable carer respite.
The role of the Dementia 1:1 Coordinator is to enable the smooth operation of the Dougie Mac 1:1 Dementia Support service through allocating Dementia Support Workers to service users, assessing service users, and maintaining effective record keeping on those who are accessing the service. The role will allow the post holder the opportunity to undertake any required training and development, with structured supervision and support. Continued professional development is supported and encouraged.
Key Responsibilities:
1. Working closely with the Dementia Operations Team Leader to enable the smooth running of the Dougie Mac 1:1 Dementia Support Service to meet organisational needs.
2. Coordinate new referrals into the 1:1 Dementia Support service, and act as the first point of contact for new referrals wishing to access the service.
3. Act as the first point of contact for Dementia Support Workers, coordinating calls as required and allocating Dementia Support Workers to service users appropriately.
4. Undertake assessments of new and current service users making individualised judgements to ensure suitability for the service. Assessments can take place onsite at the Dougie Mac Adult Hospice or in the community.
