As a Community Children's Nurse (CCN), you will provide a high level of specialist care to sick children and young people with complex health needs in the community setting. In addition to providing direct care, you will support parents and families, including teaching specific care required to maintain the child at home whenever it is safe to do so.
The CCN will provide support to patients in their home who are known to the children's acute community team and specialist nurse services, including completing wound reviews and administering home intravenous antibiotics.
You will work in partnership and liaise with the multi-disciplinary/multi-agency team.
Main duties of the job
1. Complete the Postgraduate Diploma (PGDip) Specialist Community Nursing if this has not already been completed.
2. Teach and assess families/carers to carry out specific nursing care for a variety of conditions.
3. Be responsible for assessing, implementing, and evaluating individual children's care plans.
4. Work in collaboration with parents/carers to provide care to children and young people in accordance with the child's plan of care.
5. Liaise with all members of the multidisciplinary team when changes occur.
6. Support the provision of evidence-based care for children within the community setting with the support and guidance of the senior nurses and the clinical nurse specialist team.
7. Empower children/young people, families, and carers to provide the complex care required.
8. Make informed clinical decisions using specialist knowledge and skills.
9. Maintain safe working practices when using equipment and when training others in the delivery of the child's care.
10. Co-ordinate and act as a keyworker for complex packages of care.
11. Utilise skills to effectively manage emotions in response to significant events e.g., diagnosis of a condition or end-of-life care.
12. Act as a deputy to the team lead.

Person Specification
Qualifications/training and professional development
* Registered Nurse Child, or RSCN, or Registered under sub-part 1 or sub-part 2 of the NMC register.
* Completed the Postgraduate Diploma (PGDip) Specialist Community Nursing.
* Teaching and Mentorship Course.
* Educated to Degree level.
Experience
* Experience working as a Band 5 in a Paediatric environment.
* Previous Paediatric Community Nursing experience.
* Experience of working with children with complex healthcare needs.
Skills, abilities and knowledge
* Good theoretical and practical knowledge of nursing within the clinical area of paediatrics; committed to family-centred care.
* Ability to assess, plan, implement, and evaluate care.
* Good understanding of the principles of clinical governance and clinical supervision.
* Hold a current full UK Driving Licence, with confidence to drive across a variety of rural and urban areas.
* Able to recognise situations detrimental to health and act as required.

£37,338 to £44,962 a year. Per annum, salary is dependent on experience.
