Job Overview
This is an exciting opportunity to join a new and evolving team centered around the early identification, recognition, assessment, and treatment of a deteriorating patient across CHFT.
The post holder will be a member of the Resilience, Acute Flow, and Transformation directorate sitting within the Corporate Division, working effectively with fellow ward teams and Divisional Matrons to deliver quality services for patients efficiently. There is a specific focus on the deteriorating patient and improving outcomes for patients. As part of the interview process, all candidates will have an Objective Structured Clinical Examination (OSCE) assessment in addition to the Panel interview.
The post holder will be responsible for the assessment, planning, implementation, and evaluation of care to acutely deteriorating and critically ill patients. This includes non-medical prescribing, ultrasound-guided cannulation, arterial blood gas sampling, spirometry out of hours, review of all tracheostomies/laryngectomy patients, liaising with Critical care.
Main Duties of the Job
* Provide clinical and managerial leadership for nursing and other healthcare professionals.
* Provide advice and support to wider members of the multi-disciplinary team.
* Facilitate the stepdown of patients from Critical Care to ward areas.
* Provide informal and formal education to all healthcare professionals within the Trust.
* Proactively manage the clinical workload at night as necessary.

Detailed Job Description and Main Responsibilities
* Initiate, lead, and coordinate management of patients identified as ‘clinically at risk’, referring to appropriate colleagues when necessary.
* Ensure timely and appropriate care is delivered by the relevant healthcare professionals.
* Undertake a comprehensive assessment of patients, participating in the assessing, planning, implementation, and evaluation of individualised programmes of care.
* Act as an innovative and enthusiastic role model, providing leadership, guidance, and advice to staff on clinical, operational, and professional issues.
* Demonstrate clinical expertise by role modelling patient-centred practice.
* Ensure that staff adhere to the systems and processes in place to support the provision of patient-centred care to agreed standards.
* Maintain effective communication between staff and relatives to assist in their understanding of care, treatment, and progress of the patient.
* Participate effectively in the multi-professional team providing an expert nursing perspective.
Person Specification
Knowledge, Experience & Expertise
* Extensive & recent management and leadership experience in acute/critical care settings.
* Advanced acute clinical knowledge.
* Ability to coordinate and prioritise workload.
* Excellent communication skills and ability to work within MDT.
Qualifications / Training
* Independent Prescribing or to complete this within 2 years.
* Evidence of post-registration study and development in acute or critical care.
* Teaching and assessing post-registration qualification.
* Master’s Level Qualification / working towards or equivalent highly specialised experience & knowledge.
* Evidence of leadership skills.
* Competent in cannulation & phlebotomy or willingness to undertake in 3 months.
