To assess implement and evaluate agreed care and treatment plans for each individual on the caseload which has previously been agreed with the service users / carers. To work autonomously but under the indirect supervision of a Band 7 registered nurse / team manager and in conjunction with unregistered Band 3 healthcare assistants, other health colleagues and colleagues from the Local Authority in delivery of a supportive service for those people under the care of the CMHT. There will also be an expectation of the successful candidate to work as part of our duty team, which will involve mental health assessments and providing a point of contact for existing service users in the absence of their care coordinator. The CMHT also provide a variety of clinics, including depot, Clozapine, Lithium and physical health screening clinics, which the role will also be involved with in terms of providing support for substantive clinic nurses during times of leave. Ability to travel daily between sites and patients homes in a timely manner to meet the requirements of the role/service is essential.
