Community Support Worker – Reablement
The community support service supports people to regain their independence through reablement. We are a CQC registered service that is rated overall ‘Outstanding’.
Our reablement team works with individuals to increase their ability to look after themselves following illness or injury and to rebuild the skills and confidence to help them live as independently as possible. We are working closely with colleagues and partners in health to ensure that we have an integrated and collaborative approach to our service. We are different from traditional home care, as we support and encourage people to carry out everyday activities themselves. We work closely with our occupational therapy and social work colleagues to support people coming home from hospital and support people in the community to maintain their independence.
You will play a critical role in identifying meaningful and achievable goals that matter to the person. This in turn will promote good health, wellbeing, a sense of purpose and self-worth. You will become a valued member of the team that helps people to direct their own recovery and reduce the risk of them returning to hospital.
The Community Support Service is seeking to recruit flexible, motivated and enthusiastic people who feel they can make a difference to vulnerable people living in their own homes.
You will need to have a full driving license and access to a car when at work.
We can offer a contract of guaranteed hours.
Currently recruiting to the following shifts in various locations:
1. Zone 2 = 1 x 18 hour (PM) and 2 x 21 hour (AM)
2. Zone 3 = 2 x 24 (PM) and 1 x 21 (AM)
3. Zone 4 = 2 x 18 (PM) and 2 x 21 (AM)
4. Zone 5 = 1 x 18 (PM), 1 x 24 (PM) and 2 x 21 (AM)
Community Support Workers cover a wide range of locations that are split into the following zones:
1. Zone 1: Holderness
2. Zone 2: Bridlington / Driffield
3. Zone 3: Beverley / Haltemprice
4. Zone 4: Pocklington
5. Zone 5: Goole
