The aim of the Patient Pathway Navigator role is to help support and navigate patients, their families, and carers through a cancer pathway. The service's prime aim is to be patient-focused and proactive in seeing that the patient receives the very best cancer pathway experience with continual communication and support in a timely manner.
You will be involved throughout the patient journey from referral to follow-up within the hospital setting, working directly and indirectly with patients and their carers, assisting the clinical team in coordinating the patient pathway.
The role requires someone with exceptional organisational skills who can demonstrate great communication and interpersonal skills. A caring and sensitive attitude is essential as you will be dealing with patients, their families, and carers. You will have excellent listening skills and be able to deal with people both internally and externally in a sensitive and professional manner and be confident using patient administration systems and database packages.
The post holder will be expected to work independently within the appropriate policies and procedures for the service and will actively support and contribute to the requirements of the service and ensure compliance with performance targets and quality standards required at a local and national level.
Main duties of the job
1. Communicate regularly with patients to ensure that they are aware of upcoming appointments, diagnostic tests/investigations, and operations and offer practical support with the arrangements.
2. Assist the clinical and operational teams in monitoring and supporting patients.
3. Provide basic clinical information to patients in line with the agreed pathway and learn to recognise urgent results and act upon effectively.
4. Develop and maintain effective working relationships with clinicians, CNS's, and others within department and Trust-wide service.
5. Promote dissemination of quality patient information.
6. Deal with queries about progress through secondary care in a professional and friendly manner and ensure appropriate action is taken.
7. To carry out administrative and clerical duties as requested within the department in an efficient and confidential manner.
8. Work flexibly to meet the demands of the role and requirements of the situation.
9. To be proficient in all IT systems relevant for the role.
10. Document and monitor all aspects of patient care coordination and service delivery onto the Trust IT systems to ensure the patient's progress can be measured.

Qualifications
* A good standard of general education to GCSE level (5 GCSE grade A*-C) or equivalent.
Experience
* Previous experience of booking outpatient appointments.
Knowledge
* Ability to maintain confidentiality at all times.
* Experienced iCS and ERS user.
