Social Care Assessor (Sensory)
Working Pattern: Full Time, 8:30am - 4:30pm, Monday - Friday

About the Role
We currently have an exciting vacancy on our Sensory team, which sits in our North Community Adult Social Care team, located at Trafford Town Hall. This role aims to continue building on our success and bring new experience, knowledge, and ideas to the team.
* We are recruiting a Social Care Assessor (SCA) with sensory knowledge and experience to join our Sensory team. We are looking for enthusiastic and motivated applicants who will thrive in a busy work environment.
* The successful candidate will have experience working with adults with sensory needs who may require health and social care support.
* You will manage a varied caseload, which will include Care Act Assessments, and be a part of the sensory duty rota. You will deliver exceptional social care to the adults we support and their carers, promoting choice and control as much as possible.
* A significant part of your role will be to engage in outcomes-focused and preventative conversations that build on the strengths of people and their local networks.

About you
The successful candidate will require an understanding of the assessment and review process/function, incorporating knowledge of care and support planning and its purpose. A detailed understanding of the National Eligibility Criteria under the Care Act 2014 is essential.
You will need to have experience of working with adults in need of health and social care support and sensory needs or demonstrate transferable skills. Your style and approach will reflect our EPIC values and behaviours.
You must possess good communication skills and be able to work proactively with a range of adults, applying a clear understanding of the legislative framework for Adult Social Care.
An enhanced DBS check is required. There is an expectation that you will have relevant experience and knowledge around the role and responsibilities of a SCA.

Additional Information
Flexibility will be required to work outside core hours when necessary. An enhanced DBS check with children/adults barred list check will be required. You will have flexibility in terms of hybrid working at home and in the office.
You will be required to drive in this role.
