Sedgley Coseley and Gornal Primary Care Network are offering an exciting opportunity for a Prescribing Paramedic Practitioner to join the team based at The Ridgeway Surgery (you may be asked to work at other surgeries within the PCN in the future). You must be able to work autonomously and have a caring and committed approach to patient care.
Main duties of the job
1. To assess, diagnose, treat, refer or signpost patients who attend surgery with undifferentiated or undiagnosed conditions relating to minor illness.
2. To undertake home visits, primarily for acute conditions within the boundaries of professional qualifications.
3. The post holder will use advanced clinical skills to provide education to patients, promoting self-care and empowering them to make informed choices about their treatment.

Sedgley, Coseley and Gornal Primary Care Network is a forward-thinking, innovative eight practice PCN with a combined list size of over 55,000 patients. We offer high-quality patient-centred care, whilst fostering a collaborative and supportive environment for all GPs and staff.
The successful candidate will predominately be based at The Ridgeway Surgery, the largest practice within the PCN holding a list size of over 11600. The practice is a training practice, has excellent QOF achievement and came within the top 10 of practices within the area for patient satisfaction in the 2024 national GP survey.
Job responsibilities
1. Work as part of a multi-disciplinary team (MDT) within the organisation.
2. Assess and triage patients, including same day triage and, as appropriate, provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
3. Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.
4. Use complex decision making to inform the diagnosis, investigation, and complete management of episodes of care within a broad scope of practice.
5. Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
6. Complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care.
7. Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCNs social prescribing service and, where appropriate, other community or voluntary services.
8. Be able to perform specialist health checks and reviews within their scope of practice and in line with local and national guidance.
9. Interpret ECGs.
10. Perform investigatory procedures as required and undertake the collection of pathological specimens including intravenous blood samples, swabs and other samples within their scope of practice and in line with local and national guidance.
11. Support the delivery of anticipatory care plans and lead certain services (e.g., monitoring blood pressure and diabetes risk of elderly patients).
12. Provide an alternative model to urgent and same day GP home visit for the network.
Person Specification
Experience
* Completed a two-year consolidation of learning period when a newly qualified paramedic.
* Completed further three years experience as a band 6 (or equivalent) paramedic and attained Level 7 capability in paramedic areas of practice.
* Completed and been formally signed off within the clinical pillar competencies of the Advanced Clinical Practice Framework.
* Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, PDGs, NICE and local clinical guidelines and care pathways) or make necessary referrals to other members of the primary care team.
* Able to manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.
* Use complex decision making to inform the diagnosis, investigation and complete management of episodes of care within a broad scope of practice.
* Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
* Have completed the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.
* Previously worked in general practice or within a PCN setting.
Qualifications
* Is educated to Masters degree level in relevant area of expertise.
* Has the capabilities of advanced clinical practice set out in section one of the Multi-professional Framework for Advanced Clinical Practice in England.
* Is registered with the Health and Care Professions Council (HCPC).
