Person specification Qualifications Essential criteria NVQ 3 in Health or Health and Social Care or equivalent Foundation degree or equivalent eg accredited CPD units Experience Essential criteria Experience of multi-disciplinary work. This could include substantial voluntary work or work in a related field eg housing. Experience managing own work in a team setting Experience of working with Stroke patients Experience of record keeping, report writing, correspondence Desirable criteria Experience of working as Assistant Practitioner Experience of working as Rehab Support Worker Community Working (stroke) Knowledge Essential criteria Good understanding of community care objectives and practices Understanding of role of community stroke and the social services and their services Familiarity with the role of relevant agencies. (eg health and housing) Understanding of need and risk in relation to community care Understanding of the concepts of single assessments and care planning Understanding of the roles and skills of the members of the MDT Awareness of a range of minor daily living equipment and adaptations Awareness of rehabilitation and promoting independence Desirable criteria Ability to empower patients / service users and enable them to live as independently as possible Ability to undertake standard and specialist assessments Ability to work in partnership with patents / service users and carers Skills Essential criteria Evidence of team player Effective written and verbal communication skills Ability to communicate with patients and carers who may have cognitive or communication impairment.
