Primary Care Mental Health Practitioner - Care Collaborative PCN

This is an exciting opportunity for an enthusiastic and experienced Mental Health Practitioner to join our General Practice integrated mental health service (GPimhs) in Care Collaborative PCN.
We have a Fixed Term (12 Months), Full-Time (37.5 Hrs per week - Monday to Friday 9am to 5pm) role.
The post-holder will be working from the GP Practices (as below) within Care Collaborative PCN and Gatton Place SABP Hub in Redhill.

* Moat House Surgery, Merstham, Surrey RH1 3PN
* The Wall House Surgery, Reigate, Surrey RH2 9HG
GPimhs comprise small teams based across several GP Practices aimed at addressing the needs of adults who frequently attend their GP with serious mental health needs that are not being met elsewhere.
Main duties of the job
The successful candidate will take a key role in developing and shaping a major new and transformative approach to providing mental health care within the community. They will be responsible for working closely with General Practitioners and the primary care team to provide assessments, brief interventions, and care navigation for people with significant mental health issues living in the locality. The successful candidate will need to work closely with local mental health services to support an easy in and out approach between secondary and primary care. They will have day-to-day supervisory responsibility for the Community Connector role and for managing the clinical referral priorities.
As this role is based within a community setting, you must be able to travel between multiple sites and trust locations within the set hours of your working day.

Person Specification
Qualifications
* Professional Qualification in Mental Health Nursing or Occupational Therapy or Social Work
* Professional Registration NMC, SWE or HCPC
Experience
* Completion of mentorship/preceptorship course or the intention to complete
* Experience of providing a range of evidence-based interventions to people with a variety of mental health problems and serious mental illness

Salary: £48,270 to £54,931 per annum (Includes 5% Fringe HCAS)
