Job summary
To be responsible for the delivery of evidence-based nursingservices, working as part of the practice multi-disciplinary team, deliveringcare within their scope of practice to the entitled patient population.
Working autonomously, responsible for a number of clinical areassuch as health promotion, chronic disease management, health prevention, wellwomen and well man clinics, as well as supporting the management team in thereviewing of clinical policy and procedure.
Main duties of the job
Develop,implement and evaluate individual treatment plans for acute and non-acuteconditions
Supportthe clinical team in the development of individual treatment plans for chronicdisease patients
Provideroutine nursing care to patients as required in accordance with clinical basedevidence, NICE and the NSF
Prescribemedication in accordance with PGD and within own scope of practice
Supportpatients to ensure compliance with prescribed medications
Providewound care (ulcer/Doppler, etc.) to patients
Providetravel medicine services
Requestpathology services as necessary, processing and interpreting results asrequired
Referpatients directly to other services as appropriate
Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required
Maintainaccurate clinical records in conjunction with extant legislation
EnsureSNOMED codes are used effectively
Maintainchronic disease registers
Chaperonepatients where necessary
AssistGPs with minor surgery when required
Prioritisehealth issues and intervene appropriately
Supportthe team in dealing with clinical emergencies
Implementvaccination programmes for adults and children
Liaisewith external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)

Sleights andSandsend Medical Practice is a General Medical Services Practice, located nearWhitby, North Yorkshire. It has a current list size of 5,200 patients.
The Practiceprovides services to residents of Sleights, Whitby and the surroundingvillages. It has a branch surgery at Sandsend, approximately 5 miles from themain Surgery in Sleights, along the coast. Both surgeries are purpose built,have a dispensary and parking. The Practice dispenses to 65% ofpatients.

Monitorand ensure the safe storage, rotation and disposal of medicaments

Supportand participate in shared learning within the organisation
Continuallyreview clinical practices, responding to national policies and initiativeswhere appropriate
Beaware of and contribute to the planning and implementation of Integrated Care System (ICS) (formerly CCG) patient pathwayinitiatives
Participatein the review of significant and near miss events applying a structuredapproach, root cause analysis (RCA)
