The Northumberland Recovery Partnership has an opportunity for a Band 6 Community Practitioner to work within the team.
The role entails working with those affected by substance and alcohol misuse, providing safe and effective interventions, harm reduction advice, and the use of psychosocial interventions to support service users in considering and achieving recovery.
The team consists of a well-established mix of professionals that work collaboratively to deliver individualized care. The role involves individual clinical assessment, care planning, formulation, and collaborative working across the multi-agency team.
The applicant will be expected to demonstrate leadership within the team, providing supervision and supporting students.
The applicant will be expected to cover bases that fall within the Northumberland Recovery Partnership's geographical area, which includes Blyth & Ashington.
Main Duties of the Job
Physical Health Nurse
The post holder will work within our physical health pathway within the multi-disciplinary team to provide care to those affected by substance use.
The role entails working with those affected by substance and alcohol misuse who may have a range of physical health needs. The role will be to provide safe and effective interventions, harm reduction advice, and psychosocial interventions to support service users in considering and achieving recovery.
The post holder will complete comprehensive physical health assessments in line with Trust standards and offer appropriate interventions, onward referrals, and monitoring of health. Your role will be pivotal in addressing health inequalities within areas of deprivation, delivering essential care to those whose physical health is negatively impacted by substance and alcohol misuse.
Older Persons Nurse
The post holder will work within the new Older Person's pathway within the multi-disciplinary team to provide care to those affected by substance use.
There will be an expectation of outreach support where appropriate to support older persons, whether in their home setting, a community base, or care home.

Job Responsibilities
1. To be responsible for the review and management of casework of service users who have substantial and complex needs.
2. Provide specialist clinical advice to service users, carers, staff, statutory agencies, third sector partner agencies, and the public.
3. In collaboration with the service user and carer, develop recovery and wellbeing-focused care plans, ensuring regular reviews to monitor progress and adapt the care plan as necessary.
4. To consider carer needs and ensure the carer and family are involved in care planning where appropriate.
5. To deliver evidence-based clinical and psychologically based interventions.
6. Work as a member of the multi-disciplinary team ensuring high-quality individualized care is delivered, promoting recovery and wellbeing and maximizing independence.
7. Coordinate treatment planning, interventions, reviews, and discharge planning. Provide clinical supervision to staff as delegated by the Pathway Clinical Lead and Pathway Manager.
8. To provide compassionate care based on empathy, kindness, respect, and dignity.
9. To incorporate a client-centered philosophy into nursing practice and client-centered collaborative care.

Person Specification
Education and Qualification
* Professional qualification and current professional registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
* Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
* Proven track record of post-registration clinical experience relevant to the post.
* Training in CPA (Care Programme Approach), safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and child protection.
* Up-to-date knowledge/experience of relevant legislation.
Skills and Competencies
* Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
* Risk assessment and risk management/safety planning skills.
* Demonstrate an understanding of effective and appropriate treatment options including a range of evidence-based clinical approaches as identified in N.I.C.E. guidance.
* Demonstrate effective written and verbal communication skills.
Knowledge and Experience
* Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
* 12 to 24 months band 5 qualified nurse experience dependent upon service need.
