We are seeking an enthusiastic Specialist Physiotherapist to join our Unplanned Care team based at Hemel One in Hemel Hempstead delivering care to patients closer to home and preventing unnecessary hospital admission in West Hertfordshire.
The Unplanned Care team comprises 5 pathways delivered over 7-day working: Discharge to Assess Pathway (DTA), Rapid Response (RR), Frailty Hospital at Home/virtual ward, Early intervention Vehicle (EIV) and Prevention to Admission. This role will primarily work under the Discharge to Assess and Rapid Response pathways, but may require the candidate to work across the other pathways when required. All pathways deliver nursing and therapy care in a patient's usual home environment.
Main duties of the job
RR focuses on preventing avoidable hospital admissions for patients with acute ill-health conditions by providing holistic assessment within 2 hours of referral and treatments over a 5 day pathway.
DTA supports the discharge of patients from acute and community hospitals to assess and manage rehabilitation/therapy needs to facilitate a safe discharge from hospital when the patient is medically well enough to go home. The pathway is delivered in conjunction with Adult Social Care to support the patient's reablement.
The EIV service responds to referrals for patients identified as requiring support for low complexity health and functional needs that would otherwise put them at risk of hospital admission through loss of independence and/or falls.
The Unplanned Care team works in an integrated way with the other community providers in west Hertfordshire to ensure holistic support is provided.
The Unplanned care team delivers these pathways across all 4 Herts Valleys localities (Watford and Three Rivers, Dacorum, Hertsmere, St. Albans and Harpenden) and also provides a clinical in-reach function at the acute hospitals to prevent admissions and support timely discharges.

Job responsibilities
The successful candidate needs to be a registered physiotherapist, ideally with advanced clinical skills, significant knowledge and experience in the clinical care of adults and older people in a range of healthcare settings.
Candidates should be self-motivated with strong organisational skills and able to make clinical decisions autonomously as required.
As a clinician you will use your clinical expertise, levels of judgement, discretion and decision making to support patients at home, improve patient outcomes and reduce the likelihood of unnecessary admissions to hospital.
You will be required to complete holistic assessments, giving appropriate interventions as required and have knowledge of community services to plan the ongoing care of patients.

Education/Qualifications
* Diploma/degree in Physiotherapy
* Current HCPC registration
* Evidence of up to date CPD portfolio
* Involvement in Appraisal Scheme
* Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People, Rehabilitation
* Member of Professional Body
* Member of a Special Interest Group relevant to this post
Experience
* Experience post registration as a Band 5 physiotherapist
* Experience working at a Junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory
* Experience working with voluntary organisations.

* Skills in the assessment and treatment of adults with complex multi-pathologies
